Responsibilities:
Conduct routine and follow-up visits for assisted living residents
Perform comprehensive assessments, medication reviews, and exams
Manage chronic and acute conditions
Develop and update care plans in collaboration with interdisciplinary teams
Order and interpret labs and diagnostic tests
Prescribe medications within scope of practice
Educate residents and families on health management and prevention
Maintain accurate documentation in eClinicalWorks
Participate in quality and clinical improvement initiatives
Schedule & Work Environment:
Compensation & Structure:
$135,000–$160,000 annually depending on schedule and experience
W2 or 1099 options available
Travel reimbursement included

Qualifications:
Master’s degree from an accredited Nurse Practitioner program
Active New York State NP license
DEA registration
Eligible for Medicare and Medicaid billing
Minimum of 3 years clinical experience preferred
Geriatric, long-term care, or primary care experience strongly preferred
Strong clinical judgment and patient-centered approach
