We are especially looking for individuals who are able to help serve rural counties and underserved communities.
When working with clients, you will be required to read their Individual Support Plans and find ways to help them reach their goals. In addition to working with clients, you will be required to fill out daily invoices, Daily progress notes and respond efficiently to emails and calls.
Duties:
- Provide direct support and care to individuals in various settings.
- Assist individuals with daily activities including personal hygiene, meal preparation, budgeting, socialization, communication, etc.
- Create and implement individualized care plans to meet the specific needs of each individual.
- Foster a safe and supportive environment for individuals with diverse backgrounds and abilities.
- Maintain accurate documentation of services provided and any changes in the individual's condition.
- Collaborate with a multidisciplinary team including home providers, Service Coordinators and family members to ensure the well-being of the individuals.
- Fill out daily invoices for clients
- Respond efficiently to all emails and calls/text.
- Complete Daily progress notes on time
Experience:
- Previous experience in direct support or a related field is preferred but not required. We will train on the job.
- Knowledge of HIPAA regulations and the ability to maintain confidentiality when handling sensitive information.
- Strong communication skills to effectively interact with individuals, their families, and other team members.
- Knowledge of Microsoft Office and OneDrive.
Requirements:
- Reliable Personal Transportation
- Valid Driver’s License
- Proof of Car Insurance
- Ability to pass a National Background Check
- Current CPR/First Aid or ability to get certified.
- Must be 18 years or older.
- Experience working with the IDD population preferred.
- Experience with basic computer skills
