Job Overview
We are seeking a compassionate and dedicated Personal Care Assistant to provide essential support and care to individuals in various settings, including assisted living, home health, and community care environments. Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or resident’s <PERSON>
- Monitor patient vital signs and observe for any changes in health status, reporting findings accurately while adhering to HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation
- Support medication administration following care plans and ensuring proper documentation in compliance with healthcare standards
- Maintain a clean and safe environment by performing light cleaning duties, laundry, and sanitation tasks in accordance with health protocols
- Provide emotional support through active listening, companionship, and engaging activities that promote mental stimulation and social interaction
- Document care activities meticulously, including patient observations, progress notes, and updates to care plans while respecting patient confidentiality
Skills
- Proven experience in group home settings, assisted living facilities, nursing homes, or home & community care environments
- Knowledge of Alzheimer’s care, dementia care, memory care strategies, and working with individuals with autism or disabilities
- Certified in First Aid and CPR; familiarity with HIPAAs standards for patient privacy and data security
- Ability to perform heavy lifting safely when assisting residents with mobility or transfers using equipment like Hoyer lifts
- Strong observational skills for patient monitoring and recognizing signs of distress or health deterioration
- Experience working with residents’ <PERSON> (personal preferences) to provide personalized support that respects individual dignity
- Competence in working within care plans, administering medications accurately, and following healthcare protocols for long-term or hospice care
