- Visit patients in different environments including their homes, nursing facilities, board and cares...
- Provide bedside care and monitor patient
- Follow Plan of Care and communicate with Care Coordinators and RN
- Administer and document all medication as needed
- Provide emotional support to patient and family
- Chart every hour on patients status
- Contact mortuary

Pay: $25.00 - $43.00 per hour
