Summary/Objective: The Nurse in the Louisville Health Office provides nursing care and the coordination of communications and healthcare services delivered to SCNs living in the Louisville area. This direct and indirect care may include the scheduling of healthcare appointments and physician communication as well as working with SCNs to maintain their health status. This individual will work directly with Sisters to provide clinical assessments, medical guidance and assistance with managing needs as they arise.
Duties and Responsibilities for SCNs residing at Nazareth Home:
- Coordinate new admission and when possible, have nursing staff members present for all admissions as directed.
- Oversee the care of all SCNs at Nazareth Home as directed.
- Serve as a liaison between Louisville Health Office and Nazareth Home as directed.
- Participate in Resident Care and Care Plan Meetings as directed.
Duties and Responsibilities for SCNs in the Louisville Area:
- Schedule, accompany and participate in healthcare appointments when required.
- Make hospital visits and provide follow-up during hospitalizations providing health teaching and discharge plans.
- Provide health education programming individually and with small groups.
- Assist the Office of Healthcare & Retirement in ensuring that Durable Power of Attorney for healthcare forms are updated as needed.
- Arrange for durable medical equipment and service needed to aide safe living.
- Encourage community life when working with SCNs in their living situations.
- Obtain SCN health assessments annually and with a change in healthcare status as directed.
Duties and Responsibilities of Health Office:
- As needed, attend meetings, including but not limited to the following:
- Routine meetings with OHR/Nursing
- Routine meetings with Community Service
- Collaborate with Community service as needed
- Communicate with family members as needed.
- Maintain adequate healthcare records
- Provide regular communication to Nurse Manager, regarding SCNs health status as well as any issues or concerns.
- Provide on call coverage as needed.
- Serve as a contact for external healthcare agencies.
- Assume other responsibilities within the healthcare team as needed
Other duties may be assigned.

Supervisory Responsibilities: This position has no supervisory responsibilities.
Education/Experience: Current, active and unrestricted Associate Nursing degree and/or BSN Registered Nurse licensure in Kentucky is required and 5 years’ experience or equivalent combination of education and experience. The successful candidate will demonstrate professional experience with an older adult population, strong interpersonal skills and ability to work with a wide range of age groups and personalities. Ability to be a contributing team member. Ability to manage personal stress when interacting with conflicts, illness and grief of others. Computer skills recommended to perform administrative tasks of the position, including good working knowledge of MS Work, Excel, and Power Point.
Licenses/Certificates: Current, active and unrestricted RN license in the state of Kentucky and CPR certification is required as well as a current driver’s license. Failure to renew license before expiration could result in termination of employment.
Physical Demands: The physical demands described here are represent those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to walk; use hands to finger, handle, or feel objects, tools, or controls and talk and hear. The employee frequently is required to stand, reach with hands and arms, and climb or balance. The employee is occasionally required to sit, stoop, kneel, crouch, taste or smell. The employee must frequently lift and/or more up to 15 lbs. Lifting more would be indicated only in the event of a fall by a SCN. The employee will be required to wear personal protective equipment as established in company policies and protocols. This may include gloves, face masks, face shields, goggles, gowns, and potentially other protective equipment.
Work Environment: This job operates in a home health type setting.
Position Type/Expected Hours of Work: This is a part-time position with on call.
Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
