Overview
We are seeking a dedicated and compassionate Case Manager to join our Mental Health and SUD Outpatient. In this vital role, you will coordinate patient care plans, facilitate communication among multidisciplinary teams, and ensure seamless discharge planning and utilization management. Your expertise will help improve patient outcomes, optimize resource utilization, and enhance overall patient experience. Duties
- Conduct comprehensive patient assessments to determine care needs, utilizing clinical judgment and medical terminology knowledge.
- Develop individualized care plans in collaboration with physicians, social workers, and other healthcare professionals, ensuring alignment with medical and psychosocial needs.
- Coordinate discharge planning by arranging appropriate post-acute services, home care while adhering to HIPAA regulations and Facilitiy policies.
- Perform utilization review and management by evaluating medical records for appropriateness of services.
- Monitor patient progress through EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems, updating case files regularly.
- Facilitate intake processes for new patients, ensuring accurate documentation of medical history and current condition.
Requirements
- Knowledge of medical terminology, case management and utilization review protocols.
- Experience with social work or behavioral health services is highly desirable.
- Familiarity with KIPU EMR/EHR systems and compliance with HIPAA regulations.
- Excellent discharge planning skills along with managed care knowledge to optimize patient outcomes efficiently.
- Ability to handle complex cases involving Mental Health issues, depression, PTSD.
- Strong communication skills paired with a proactive approach to patient service and interdisciplinary collaboration.

Pay: $22.00 - $26.00 per hour
