Position Overview
Coordinate patient referrals in transition to home care services and provide assistance with the collection of pertinent medical and pharmaceutical data related to the administration of intravenous therapies in the home.

Position Requirements:
High School Diploma or equivalent.
Minimum of one-year experience in oral and infusion pharmacy desired,
Excellent organizational and interpersonal skills.
Basic knowledge or familiarity in medical terminology and diagnostic codes.
Basic knowledge or familiarity with insurance verification, healthcare benefits assessment and paperwork.
Bilingual is a must.
Duties and Requirements
1. Assists in the coordination of patient care with ancillary personnel.
2. Present a positive professional image and promote the company in all work situations.
3. Develops a good working relationship with employees, patients, families, physicians, nurses, and other referral sources.
4. Correlates referral information on new patients in order to streamline communication of data related to the plan of care, administration of home therapies.
5. Obtains authorization for services as needed for payors.
6. Gather pertinent clinical information in and disseminate to clinical staff, enter all information necessary in the computer to produce a complete patient record.
7. Obtains weekly clinical reports and obtains authorization extensions as necessary.
8. Participates in the daily huddles (morning and evening) with pharmacy staff, including the pharmacist.
9. Participates in the coordination of deliveries to the patients home to ensure they are cared for in an efficient and convenient manner.
10. The referral of patients to a home health agency, etc. as necessary
11. Participate in departmental PI/QI activities as necessary.
12. Ensure compliance with the organization's Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information.
13. Perform any additional duties as assigned to meet the needs of the department and organization.
