Country Club Rehab & Healthcare is committed to excellence and is seeking a dedicated MDS / Care Plan Coordinator to join our team.
Participate in developing comprehensive, individualized care plans for each resident
Ensure resident care plans are completed, updated, and reviewed in accordance with RAI guidelines and changes in resident needs
Manage and implement the Clinical and Prospective Payment System (PPS) MDS completion schedule
Direct, organize, and schedule required resident assessments and related documentation for Medicare, Medicaid, managed care, and HMO reimbursement
Coordinate and oversee all aspects of the MDS assessment process to ensure compliance with federal and state regulations
Participate in pre-admission evaluations to determine potential residents’ level of care
Assist with level-of-care determinations upon admission and monitor changes requiring timely MDS assessments
Review MDS assessments and related documentation to ensure accuracy and timely completion
Monitor reports related to MDS and care plan compliance and follow up as needed
Analyze Quality Indicator reports and ensure findings are reflected in resident care plans
Maintain current knowledge of state and federal regulations related to RAI, care planning, and PPS
