Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for assessing, developing, and implementing individualized treatment plans to improve patients' mobility, strength, and overall functional ability. You will work closely with a multidisciplinary team to deliver high-quality patient care across various settings, including outpatient clinics, inpatient facilities, and home health environments. Your expertise will help patients recover from injuries, surgeries, or manage chronic conditions, empowering them to regain independence and improve their quality of life.
Responsibilities
- Conduct comprehensive patient evaluations to determine physical therapy needs, including assessments of physiology and anatomy
- Develop personalized treatment plans that align with clinical documentation standards and best practices in physiotherapy
- Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs tailored to individual patient goals
- Collaborate with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate care plans and discharge strategies
- Document patient progress accurately using electronic health records (EHR) systems and ensure compliance with medical terminology standards
- Provide patient education on treatment procedures, home exercises, and community resources to support ongoing recovery
- Manage discharge planning processes for patients transitioning from inpatient care to home or community settings
Skills
- Extensive knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practice
- Proven experience working with geriatric patients, pediatrics, neurology cases, orthopedics, and outpatient rehabilitation centers
- Strong skills in developing effective treatment plans based on individual assessments and clinical observations
- Proficiency in outpatient clinic experience, inpatient hospital settings, and home health environments
- Ability to utilize EMR (Electronic Medical Records) systems efficiently for documentation and care coordination
- Excellent client communication skills to foster trust and motivate patients throughout their rehabilitation journey
- Familiarity with discharge planning procedures and working collaboratively with healthcare teams to optimize patient outcomes
