Position Summary
We are seeking a compassionate, experienced Registered Nurse (RN) to provide clinical oversight, nursing supervision, and delegation for our in-home care services. The RN will play a key role in ensuring safe, high-quality, client-centered care while maintaining compliance with Oregon Health Authority (OHA) In-Home Care Agency Rules and Oregon State Board of Nursing (OSBN) requirements.
This position is ideal for an RN who enjoys working independently, supporting caregivers and clients, and serving as a clinical resource within a collaborative home care environment.
Key ResponsibilitiesClinical Oversight & Nursing Delegation
- Provide nursing supervision and clinical oversight of caregivers and CNAs.
- Perform nursing delegation in accordance with OSBN requirements.
- Assess clients to determine the appropriateness of delegated nursing tasks.
- Provide instruction, supervision, and follow-up for delegated tasks.
- Conduct reassessments as required based on client condition and delegation requirements.
- Ensure caregivers perform assigned tasks safely and within their scope of practice.
Client Assessment & Care Coordination
- Conduct initial and ongoing nursing assessments as required.
- Participate in the development and review of service plans when nursing involvement is indicated.
- Monitor changes in client condition and respond appropriately to health concerns.
- Communicate with clients, families, healthcare providers, and agency staff regarding care needs.
- Provide clinical guidance that promotes safe, individualized, and client-centered care.
Training & Staff Support
- Provide education and training related to delegated nursing tasks.
- Participate in staff orientation and competency evaluations.
- Serve as a clinical resource for caregivers, CNAs, and administrative staff.
- Promote infection control, safety, and best practices in home care.
Quality Assurance & Risk Management
- Participate in the agency's Quality Assessment and Performance Improvement (QAPI) program.
- Review incidents, complaints, and clinical concerns for trends and opportunities for improvement.
- Support corrective actions and quality improvement initiatives.
- Promote client safety, dignity, privacy, and rights.
Communication & Collaboration
- Collaborate closely with the Administrator regarding clinical operations and regulatory compliance.
- Communicate professionally and effectively with clients, families, staff, healthcare providers, and regulators.
- Represent the agency with professionalism, integrity, and respect.
Qualifications & Requirements
- Current, active Oregon Registered Nurse (RN) license in good standing.
- Understanding of client rights, confidentiality, and HIPAA requirements.
- Ability to successfully complete a criminal background check and fitness determination.
- Strong clinical assessment, documentation, organization, and communication skills.
- Ability to work independently and exercise sound clinical judgment.
- Ability to educate, supervise, and support unlicensed caregivers.
Preferred Qualifications
- Experience in home care, in-home care, home health, hospice, or long-term care.
- Previous experience with nursing delegation and caregiver supervision.
- Familiarity with OHA licensing, surveys, inspections, and regulatory requirements.
- Experience with QAPI or quality improvement programs.
- Training or experience in chronic disease management, dementia care, or palliative care.
