What You'll Do
- Provide comfort-focused personal care, including ADLs, grooming, hygiene, and mobility assistance.
- Assist with comfort measures and medications as directed by the clinical team.
- Observe and communicate changes in patient condition.
- Provide emotional support and companionship to patients and families.
- Collaborate with nurses, social workers, chaplains, and the interdisciplinary care team.
- Help maintain a safe, peaceful, and comfortable environment during every visit.
- Follow individualized hospice care plans with compassion and consistency.
What We're Looking For
- Active Minnesota Certified Nursing Assistant (CNA) certification.
- Experience in hospice preferred.
- Candidates with backgrounds in Palliative Care, Oncology, Home Health, Assisted Living, Long-Term Care, or Skilled Nursing are encouraged to apply.
- Compassionate, dependable, and emotionally resilient.
- Strong communication skills and a collaborative approach.
- Valid driver's license and reliable transportation.
