Day shift schedule: 8:00 AM – 4:30 PM

12-week contract

May be required to float within skill set on-site or to facilities within 40 miles

Active WI or compact RN license required

BLS certification required (AHA-issued)

Minimum 1 year of care management experience required

Must possess a US Social Security Number and provide a US home address

Provide care management services within the Case Management unit
