View Point Health proudly provides outpatient treatment to children, adolescents and adults. At the outpatient centers, we promote overall health and quality of life for individuals, youth and families through evidenced-based therapies and medication management to reduce the impact of mental illness, behavior challenges, and substance use for individuals, youth, and families.
Under general supervision, performs psychosocial assessments, treatment planning/monitoring and discharge planning. Provides counseling/therapy to clients requiring complex intervention. Performs commitment determinations and/or executes orders of involuntary commitments.
Duties & Responsibilities:

Provides comprehensive mental health and addictive disease assessments for consumers seeking treatment services.

Provides professional evidenced-based therapy services to children and adolescents with behavioral health concerns or disorders.

Conducts face-to-face (or via telehealth) comprehensive assessment to identify problems and service needs.

Provides play therapy, family counseling and skill development sessions as indicated in the service plan.

Monitors client progress and compliance with the service plan

Provides crisis intervention services to existing and walk-in clients as needed.

Provides on-call, after-hours services on a rotational schedule (approximately one week every 3 months). Assesses need for emergency admission certification.

Executes the legal forms required to refer the client to an emergency receiving facility for evaluation of the need for involuntary commitment to a hospital or crisis intervention facility.

Arranges transfer of the client to the receiving facility.

Writes individual service plans that meet Medicaid and other required standards for diagnoses, content, justifications for treatment, evidence of client input, and treatment time frames.

Writes service plans that are clear, concise, complete, grammatically correct, timely, and properly coded.

Secures or provides necessary signatures required to secure authorization for services.

Follows all Medicaid, state, insurer, and other regulations related to the credentials required to provide authorized services.

Writes timely progress notes that relate directly to the treatment outcomes described in the service plan. Assembles, analyzes, and interprets new/updated information about the client in order to reassess treatment needs.

Identifies unmet needs, reviews available resources, and modifies the treatment plan as indicated. Requests treatment authorizations and re-authorizations within required time frames.

Provides and bills only those client services that are authorized in the service plan.

May serve as case manager to facilitate and ensure communication, linkage, and continuity of care between clients, physicians, other organizational components, community resources and service providers.

Establishes linkages and coordination of services with community agencies.

Develops and maintains cooperative working relationships with community service providers.

Informs clients and families of the availability of community advocacy and support groups.
