OPEN SCHEDULES:
Monday- Friday: 8am-2pm & 2pm- 10pm
Saturday/Sundays: 8am-8pm & 8pm-8am
Responsibilities
- Provide direct personal care assistance, including help with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patients’ health status by observing vital signs, mental state, and overall condition; report any concerns promptly
- Assist with medication administration following care plans and HIPAA regulations to ensure confidentiality and safety
- Support residents with meal preparation, feeding, and nutritional needs while maintaining cleanliness and hygiene standards
- Utilize patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe mobility and transfers
- Maintain detailed documentation of patient progress, care plans, and incident reports in compliance with healthcare regulations
- Facilitate patient engagement through activities that promote mental stimulation and social interaction for individuals with autism, dementia, or neurodevelopmental disorders
Skills
- Experience working with individuals with developmental disabilities, intellectual disabilities, or neurodevelopmental disorders such as autism or Alzheimer's disease
- Proficiency in patient observation, monitoring vital signs, and recognizing signs of distress or deterioration
- Knowledge of medication administration protocols and first aid/CPR certification for emergency response preparedness
- Ability to perform heavy lifting safely using proper manual handling techniques; experience with patient transfer equipment like Hoyer lifts is preferred
- Strong communication skills to effectively interact with clients, families, healthcare teams, and community resources
- Familiarity with care plans development and implementation tailored to individual needs in settings such as nursing homes or home health environments
- Competence in maintaining HIPAA compliance while managing sensitive patient information
