You’re calm under pressure, direct when needed, and lead with a compassionate heart. You can handle hard conversations, you document cleanly, and you don’t let discharge planning drift.
- Lead psychosocial assessments, care planning, and ongoing support for residents and families
- Coordinate safe, appropriate discharges (and document the “why” behind the plan)
- Manage grievances/complaints professionally and ensure follow-up is completed and tracked
- Support residents’ rights, advance directives, and decision-making resources
- Collaborate daily with Nursing, Admissions, Therapy, and the Business Office to remove barriers
- Assist with behavior support plans, mental health coordination, and community resource referrals
- Maintain accurate, survey-ready documentation that reflects resident needs, preferences, and interventions
- Participate in QAPI and support investigations as needed (abuse/neglect allegations, elopement risk, etc.)
- Discharges are planned, safe, and documented—not rushed, messy, or last-minute
- Families feel informed and respected (even when they don’t like the answer)
- Concerns don’t sit—grievances are owned, addressed, and closed out
- Surveyors can open your documentation and clearly see: assessment plan action follow-up
- Bachelor’s degree in Social Work or related field required (MSW preferred)
- Skilled Nursing Facility experience strongly preferred
- Strong written documentation skills and comfort with difficult conversations
- Organized, consistent follow-through, and able to work fast without losing accuracy
