JOB DESCRIPTION
JOB TITLE: Physical Therapist
JOB SUMMARY: Provides Physical Therapy in accordance with the orders and the plan of care of the attending physician or podiatrist. The physical therapist utilizes diagnostic and prognostic muscle, nerve, joint, and functional ability tests to develop and provides therapeutic or maintenance programs in the home environment.
REPORTING STRUCTURE: Director of Patient Care Services for overall clinical performance. Coordinates with RN assigned to case if assigned as Case manager.
DUTIES:
1. Evaluates the patient's home for safety and the patient's present level of functioning, incorporating age related factors/criteria.
2. Assists in developing and updating the plan of care, and documents diagnostic findings, therapy initiated and progress towards goals in patient's medical record.
3. Submits progress summaries to physician every sixty-(60) days and as requested.
4. Utilizes physical therapy modalities as an integral part of the therapy program.
5. Advises, consults and when appropriate, instructs the patient, family, and other agency personnel regarding the patient's therapy program,
6. Teaches and supervises other health personnel, where appropriate,
7. Evaluates the home environment and makes appropriate recommendations,
8. Completes clinical and progress notes and other required/requested documentation
9. Provides care following all infection control and exposure control policies, as applicable,
10. Observes, records, and reports to the physician the patient's response to treatment and any changes in the patient's condition,
11. Coordinates the patient's physical therapy needs with other agency services,
12. Supervises patient services of Home Health Aides when indicated or appropriate,
13. Instructs/counsels family members and caregivers in patient's therapy program, when appropriate,
14. Identifies the need for, assists the patient in obtaining, and instructs in the use of prosthetic and orthotic devices and durable medical equipment,
15. Evaluates the home environment and makes recommendations as appropriate,
16. Participates in Utilization Review as requested and attends or provides verbal/written input to case conferences,
17. Provides and participates in orientation and in‑service educational programs for the Home Health Agency staff, and
18. May contribute to staffing and equipment budget planning.
19. Submits weekly schedule on Fridays for following week and updates clinical management staff regarding any change in schedule.
20. Complies with HIPAA privacy and anti-fraud and abuse requirements.
21. Performs initial and ongoing assessments to determine level of functioning, including OASIS assessments at appropriate time points
QUALIFICATIONS:
Currently licensed by Physical Therapy Committee of the California Board of Medical Quality Assurance.
PHYSICAL REQUIREMENTS:
Must be able to see, hear and speak without limitation for documentation, reports and other written or telephone communications, Must be able to sit for long periods and lift a minimum of ten pounds.
