The Brookwood Community is looking for a Transition Assistant in Work Program. This position transports Citizens around campus; assists with toileting, hygiene, and any other transportation needs by the Citizens. During open time, they support the Work Program Supervisors in their efforts to teach and stimulate functionally disabled adults to learn and develop artistic and manual skills. This position assists Citizens in their daily work routines, personal hygiene needs, mealtime activities, behavioral interventions, and to utilize TEACCH and best practices whenever possible in the workshop environment. This position also assists with daily Tram Duty. They will assist in helping to create a positive work environment that enhances the Citizens' sense of independence and self-worth while adhering to Brookwood's Policies and Procedures and Core Values.
I. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Other duties may be assigned. Transport Citizens to and from their Homes and Shops before and after work; making sure they bring with them all their personal items. Transport Citizens during the work hours as needed to Cafe.
Assists Citizens with toileting needs, using lifts as necessary, pick up a change of clothing from the Homes, if needed; transport Citizens as directed by Work Program Director/Asst. Director of Work Program
Helps Citizens by going and getting clothing as needed from homes on campus. Helps assist with showers as needed during the workday for any Citizen that may have had an accident. Helps staff with changing clothes for Citizens, as needed.
Works to complete the cafetorium set up each day for the lunch periods by setting up placemats, plates, silverware, etc. Utilizes Citizens to help with this.
Must be able to follow all required hygiene practices as per the guidelines. Use/ wear gloves, disinfectant,
Assures dietary needs and personal hygiene issues of the Citizens are met, as needed. Helps the Medical Staff by assuring on-going medically related activities are completed, specifically for those Citizens who require daily treatment programs.
Report any Citizen’s complaints of pain and/or exhibition of unusual behavior to Medical Staff and on-call supervisor.
Supports the transition of Residential and Day Citizens to another shop or scheduled activity by assuring that their belongings, proper diets, and snacks move with them. (In cold weather double check for any hats, gloves, or coats.)
Participates in activities with the Citizens during work hours, including getting in the pool and going swimming with the Citizens during the summer for “Cool in the Pool”.
Participates in cross-training in multiple shops, as needed; Supervisors or Supervisor Assistants may be moved to another shop as directed by the Shop/Studio Lead or Director of Work Program.
Works safely and looks out for the safety of fellow staff/Community Members and Citizens. Strictly follows all safety practices and procedures; reports any unsafe conditions or actions to your supervisor, Shop/Studio Lead, or the Community’s Safety Coordinator. If at any time you do not feel you have the capabilities or the proper training to handle a situation in your shop/studio, please contact your supervisor immediately.
II. QUALIFICATIONS – To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education and Experience – Should possess (but not required) experience in direct care work as a Resident Assistant or as a CNA in a nursing home or hospital setting. Knowledge of basic first aid, safety, basic hygiene, and CPR training helpful but not required. High School diploma/GED and some college courses; additional job experience in this field is not required but helpful.
Skills and Abilities – Possess personal characteristics such as patience, tolerance, and a willingness to learn more about specific disabilities and adapt to expectations. Incumbents must have a strong sense of serving and caring for the needs of adults with functional disabilities. In addition, incumbents must possess basic written and verbal language skills and be familiar with basic computer operations and email or Internet procedures. High school diploma with some college courses preferred.

While performing the duties of this job, the employee is regularly required to stand, walk, and talk while being aware of Shop/Studio environment. The Transition Assistant frequently is required to reach with hands and arms, stoop, transfer Citizens who are wheelchair bound, kneel, or crouch. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision, color vision, and depth perception.

The noise level in the work environment, especially in the break area, is considered moderate.
While performing the duties of this job, the employee is frequently exposed to wet and/or humid conditions, fumes or airborne particles, and outside weather conditions. The employee is occasionally exposed to chemicals, paints, extreme cold and extreme heat.
