Job Summary
Join our dedicated healthcare team as a Home Health Registered Nurse, where you will deliver compassionate, patient-centered care directly in patients’ homes. Responsibilities
- Conduct comprehensive patient assessments to evaluate health status, vital signs, and physiological needs in home settings.
- Familiar with Oasis documentation.
- Develop and implement individualized care plans aligned with medical orders and patient goals.
- Manage post-acute care transitions by coordinating discharge planning and ensuring continuity of care.
- Provide skilled nursing interventions including wound care, medication management, IV therapy, and infectious disease care.
- Collaborate closely with healthcare professionals such as physicians, therapists, social workers, and case managers to optimize patient outcomes.
- Utilize electronic health records (EHR) systems for accurate documentation of medical records, patient progress notes, and clinical case management activities.
- Educate patients and their families on health conditions, medication adherence, safety protocols, and self-care techniques to promote independence.
Qualifications
- Valid Registered Nurse (RN) license with current state certification.
- Demonstrated experience in home health nursing or related outpatient settings; inpatient or hospital experience is highly valued. 1 year experience required.
- Strong understanding of medical terminology, physiology, HIPAA regulations, and health regulation policies.
- Proficiency in EMR/EHR systems for documentation and patient record management.
- Excellent patient service skills with the ability to communicate effectively with diverse populations including geriatrics and pediatric patients.
- Ability to perform detailed patient assessments and manage complex clinical cases involving multiple comorbidities such as behavioral health or infectious diseases.
- Experience managing patients through case management strategies including utilization management and diagnostic evaluation processes.

Pay: $60.00 - $75.00 per hour
