Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices
- Administer medications and manage medication schedules in compliance with healthcare protocols and HIPAA regulations
- Support patients with specialized care needs such as Alzheimer’s disease, dementia, or developmental disabilities through tailored caregiving strategies
- Document medical records and care notes using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems with precision and confidentiality
- Assist with meal preparation, feeding, laundry, cleaning, and maintaining a safe and hygienic environment in accordance with home & community care standards
- Collaborate with teams to ensure comprehensive patient care plans are followed effectively
Qualifications
- Valid CNA certification or Home Health Aide license required; prior experience in nursing homes or long-term care settings highly preferred
- Demonstrated experience working with individuals with disabilities or in assisted living environments
- Ability to perform patient observation accurately and respond promptly to changing .
- Proficiency in using EMR/EHR systems for documentation; familiarity with HIPAA regulations to ensure patient privacy
- CPR certification required; additional certifications such as Alzheimer’s/dementia training are a plus
- Strong communication skills, empathy, patience, and a proactive attitude towards patient safety and comfort

Pay: $14.00 - $20.00 per hour
