Job Overview
We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) on call to provide vital patient care and support during after-hours and emergency situations. As an on-call RN, you will be the primary point of contact for urgent patient needs, coordinating care plans, assessing symptoms, and collaborating with interdisciplinary teams to optimize patient outcomes. Your expertise in critical care, hospital medicine, and post-acute care will be instrumental in managing complex cases with professionalism and empathy.
Duties
- Respond promptly to after-hours patient calls, providing expert nursing advice and emotional support.
- Conduct thorough patient assessments via phone or in-person visits, including vital signs, symptom management, and physiological evaluations.
- Coordinate with physicians, hospice team members, and emergency responders to facilitate appropriate interventions and ensure seamless continuity of care.
- Assist with discharge planning, medication management, and documentation within medical records while adhering to HIPAA regulations.
- Monitor patient status closely during acute episodes or emergencies, utilizing your knowledge of infectious disease care, ICU protocols, and trauma center procedures.
- Manage intake processes for new patients requiring urgent hospice services or post-acute care transitions.
- Document all interactions accurately in medical records and communicate updates effectively to the interdisciplinary team.
Experience
- Proven experience in critical care settings such as ICU, Emergency Room, Level I or Level II Trauma Centers, or Medical-Surgical units.
- Prior involvement in hospital medicine or outpatient clinic environments with a focus on patient service and assessment.
- Strong background in post-acute care including hospice, geriatrics, behavioral health, or palliative settings.
- Familiarity with medical terminology, physiology knowledge, diagnostic evaluation procedures, and utilization management practices.
- Experience working with diverse patient populations such as pediatrics (PICU), adults in inpatient or outpatient settings, or post-acute care environments.
- Demonstrated ability to handle high-pressure situations with professionalism while maintaining excellent patient service standards.

This On call Nurse Position will be paid at a per visit basis at $200 per visit.
Pay: $84,071.81 - $110,000.00 per hour
