Are you a compassionate and skilled LPN looking to make a meaningful impact in patients’ lives from the comfort of their homes?
We are seeking a dedicated Home Health LPN to join our team. This full-time role offers the chance to deliver personalized care, build lasting relationships, and work independently while being supported by a collaborative clinical team. The LPN would be float between Lehigh and Northampton counties.
Key Responsibilities:
- Deliver skilled nursing care to patients in their homes
- Administer medications, treatments, and wound care
- Monitor patient conditions and report changes to the supervising RN or physician
- Educate patients and families on care plans and health management
- Maintain accurate and timely documentation in EMR systems
Qualifications:
- Active LPN license in Pennsylvania
- Minimum of 1 year clinical experience (home health preferred)
- Strong communication and critical thinking skills
- Reliable transportation and willingness to travel locally
- CPR certification required
- Willing to do on call rotation during the weekends
