Yavapai County Community Health Services is seeking a dedicated Staff Physician to provide high-quality, compassionate care in a collaborative primary care setting. The ideal candidate will have experience in OB-GYN or reproductive health and a passion for serving diverse populations. Bilingual in Spanish skills and knowledge of or experience working in a Federally Qualified Health Center (FQHC) environment are highly preferred. - Provides comprehensive medical care in accordance with regulations and scope of practice for licensure in Arizona, State, Federal and other grant requirements, and the requirements of all payors.

Examines patients and conducts appropriate tests to determine medical condition/health status, determines and implements treatment plans and/or refers patients to specialists and other medical facilities as needed.

Supervises medical team, including review and co-signing of patient record entries by mid-level providers.

Documents all medical services and patient interactions into the Electronic Health Record in accordance with department standards, all federal reporting requirements, (Uniform Data System), Meaningful Use criteria.

Maintain an efficient patient workflow within the designated template.

Meets Patient Centered Medical Home and standards of the quarterly Quality Improvement audits including in Chronic Medical Conditions such as Diabetes (DM) and Hypertension (HTN).

Provides leadership and direction in the improvement of one of the Clinical Performance Measures and participates in the other Quality Improvement Program initiatives, as requested.

Participates in on call duty, as required.

Represents the department in community outreach events such as health screenings and physicals at Health Fairs, school sports physicals and community sports physicals.

Responds to community emergencies with a public health consequence according to the department’s Emergency Response Plan.
