About Us
Supportive Care Specialists is a growing supportive care group partnering with hospitals, LTACs, skilled nursing facilities, assisted living communities, and other healthcare organizations throughout Southern California. Our mission is to support patients, families, and healthcare partners by delivering compassionate, high-quality clinical services while building lasting relationships across the communities we serve.
As we continue to expand, we're creating a new Clinical Liaison position to strengthen our presence throughout Southern California. Because this is a newly established role, finding the right person is our highest priority. We're looking for someone who will represent our organization with professionalism, integrity, and enthusiasm while becoming a trusted partner to the facilities and providers we serve.
About the Role
This is a field-based position designed for someone who enjoys being out in the community rather than behind a desk.
You'll spend your time traveling between hospitals, LTACs, skilled nursing facilities, and other healthcare partners, building relationships with physicians, case managers, discharge planners, social workers, nursing leadership, and administrative teams. Every day will be different, with opportunities to visit facilities, strengthen partnerships, identify new referral opportunities, and serve as the face of Supportive Care Specialists.
The ideal candidate is comfortable with frequent travel throughout Southern California and enjoys meeting new people, developing professional relationships, and working independently across multiple locations.
Responsibilities
- Build and maintain strong relationships with hospitals, LTACs, skilled nursing facilities, assisted living communities, physicians, case managers, discharge planners, social workers, and healthcare leadership.
- Travel regularly throughout Southern California to visit referral partners and maintain a visible presence within assigned facilities.
- Educate healthcare professionals about Supportive Care Specialists and the services we provide.
- Identify opportunities for collaboration and facilitate appropriate patient referrals.
- Partner closely with clinical and operational teams to ensure smooth communication and continuity of care.
- Represent the organization at facility meetings, networking events, and community outreach opportunities.
- Track referral activity, maintain strong facility relationships, and help support continued organizational growth.
Qualifications
- Clinical healthcare experience preferred.
- Experience working in hospitals, LTACs, skilled nursing facilities, post-acute care, hospice, supportive care, case management, physician relations, or healthcare business development is highly desirable.
- Excellent communication, presentation, and relationship-building skills.
- Outgoing, personable, and confident interacting with physicians, executives, and interdisciplinary healthcare teams.
- Highly organized with the ability to work independently in a field-based environment.
- Valid California driver's license and reliable transportation.
- Ability and willingness to travel extensively throughout Southern California.

Pay: $65,000.00 - $85,000.00 per year
