Responsibilities
As a Home Health RN, you will:
- Perform comprehensive patient assessments and develop individualized plans of care.
- Provide skilled nursing services in patients' homes.
- Monitor vital signs, medications, symptoms, wounds, and overall clinical condition.
- Provide wound care, medication management, injections, catheter care, and other skilled nursing services as ordered.
- Educate patients and family members regarding medications, disease management, safety, nutrition, and care needs.
- Coordinate care with physicians, therapists, home health aides, and other members of the interdisciplinary team.
- Communicate changes in patient condition to physicians and appropriate clinical team members.
- Complete accurate and timely clinical documentation using the company's electronic medical record system.
- Participate in care planning and help patients safely remain at home whenever possible.
- Maintain compliance with Medicare, HIPAA, agency policies, and applicable state and federal regulations.
Qualifications
- Current and unrestricted Ohio Registered Nurse (RN) license.
- Current BLS/CPR certification.
- Previous home health experience is preferred but not required for the right RN candidate.
- Experience in hospitals, skilled nursing, rehabilitation, assisted living, hospice, or other clinical environments is welcomed.
- Strong assessment and critical-thinking skills.
- Excellent communication and patient-education skills.
- Comfortable working independently in patients' homes.
- Reliable transportation and ability to travel within the assigned West Cleveland territory.
- Strong organizational and time-management skills.
- Comfortable using electronic medical record systems and mobile technology.
