Description:
Seeking part time LMSW as a Hospice Social Worker. Medical social services will be provided by a qualified social worker, under the direction of a physician. Social work services will be based on the patient's psycho social assessment and the patient and family's needs. The social worker participates in the coordination of care with the Interdisciplinary Group (IDG) and participate in on-call rotation as the Agency needs.
Requirements:
Education: Masters Degree in Social Work from a State accredited program.
Licensure: Texas Certification; Current Driver's License
Experience: Minimum one (1) year experience in a health care agency. Hospice preferred.
Skills: Working knowledge of community resources. Good interpersonal skills.
Transportation: Reliable transportation. Valid auto insurance.
