Job Summary
We are seeking a dedicated and highly skilled Registered Nurse to join our team. The ideal candidate will possess extensive clinical expertise, a passion for client-centered care, and the ability to adapt to diverse healthcare settings. As a Registered Nurse with Home Health Companions, you will be responsible for delivering comprehensive nursing care in clients' homes, coordinating treatment plans, and ensuring adherence to healthcare standards. Responsibilities
- Provide direct client care across various settings, including home health, assisted living, long-term care, and outpatient clinics.
- Makes the initial evaluation visit and regularly re-evaluates the client’s nursing needs.
- Initiate the plan of care and make necessary revisions collaborating with multidisciplinary teams including physicians, therapists, and social workers.
- Provide support for those services requiring substantial and specialized nursing skills.
- Administer medications, injections, and perform wound care with precision and adherence to aseptic techniques as required.
- Manage complex cases involving chronic conditions such as cardiac issues, respiratory therapy (ventilators), dialysis, feeding tubes, or infectious disease care.
- Prepare clinical and progress notes and summaries of care.
- Inform the physician and other personnel of changes in the patient’s condition and needs.
- Educate patients and their families on disease management, medication administration, nutrition, and self-care techniques.
- Supervise and teach other nursing personnel as needed.
- Assist, help plan, and implement in-service and training programs.
- Oversee the maintenance of clinical records, statistics, reports, and records for purposes of evaluation and reporting of organizational activities.
- Assure proper maintenance of clinical records in compliance with local, state, and federal laws (HIPAA/OSHA Compliance).
- Assure compliance with all local, state, and federal laws regarding licensure and certification of organization personnel.
- Stay informed about changes in the field of nursing and home health care and share information with appropriate personnel.
- Promote and follow administrative policies to ensure quality of care.
- Must have the ability to demonstrate solid judgment and discretion, and a sense of understanding and urgency for priorities.
- Requires the ability to prioritize and complete work on time, be detail-oriented, exhibit a service orientation to both staff and clients and be a professional team player with a positive attitude
Qualifications:
- - Education: Bachelor’s degree in nursing (BSN) required; Bachelor’s Degree in Nursing. Must have graduated from an accredited program by the National League of Nursing.
- - Licensure: Active RN license in the state of Texas or Compact License with current unencumbered licensure to practice professional nursing in the state of Texas.
- - Minimum five years of clinical experience with two to three years working in home health or related health care organization.
Skills:
- - Excellent leadership, communication, and delegation skills.
- - In-depth knowledge of home health regulations and compliance requirements
- - Ability to accurately assess client needs, develop protocols, create and monitor quality care plans
- - Ability to work effectively in a fast-paced and dynamic environment
- - Minimum three to five years of direct employee supervision, including both exempt and non-exempt personnel.
- - Possess exceptional nursing assessment skills.
- - Customer service experience and ability to deal tactfully with clients, staff, and community.
- - Must be able to pass a background check.
- Preferred:
- - Case management skills.
