Job Responsibilities
- Develop, implement, and monitor individualized patient care plans
- Coordinate care between patients, families, healthcare providers, specialists, and community resources
- Conduct clinical assessments and evaluate patient progress
- Facilitate discharge planning and transitions of care
- Advocate for patients to ensure timely access to appropriate healthcare services
- Collaborate with physicians, nurses, social workers, and interdisciplinary care teams
- Support utilization review, resource management, and quality improvement initiatives
- Educate patients and families regarding treatment plans, healthcare resources, and disease management
- Promote continuity of care and positive patient outcomes across healthcare settings
- Maintain accurate documentation and compliance with facility and regulatory requirements
Minimum Qualifications
- Active Registered Nurse (RN) license
- Current BLS certification
- Current ACLS certification
- Minimum of 2 years of recent Case Management RN experience
- Strong communication, critical thinking, and care coordination skills
- Ability to work effectively in fast-paced healthcare environments
