Position Summary
We are seeking dedicated and compassionate Direct Care Workers / Direct Support Professionals (DSPs) to provide hands-on support to individuals with intellectual and developmental disabilities (IDD), physical disabilities, or other support needs in their homes and community settings. DSPs play a vital role in the lives of the individuals we serve — supporting daily living activities, promoting independence, and helping individuals achieve their personal goals. Key Responsibilities

Provide direct, person-centered support to individuals with IDD, physical disabilities, or behavioral health needs in home and community settings

Assist individuals with activities of daily living (ADLs) including bathing, grooming, dressing, meal preparation, eating, and mobility

Support individuals in community integration activities such as shopping, recreational outings, appointments, and social events

Implement Individual Support Plans (ISPs) and Behavior Support Plans (BSPs) as directed by the care team

Assist with medication administration or reminders in accordance with agency policy and Pennsylvania regulations

Use Electronic Visit Verification (EVV) systems to accurately clock in/out and document service delivery per OLTL and ODP requirements

Maintain accurate and timely documentation of daily activities, behavioral observations, and any incidents or changes in condition

Report changes in an individual's health, behavior, or environment to the supervisor or clinical team promptly

Participate in required agency training, in-services, and competency evaluations

Treat all individuals with dignity, respect, and cultural sensitivity at all times

Adhere to all QHCG policies and procedures and applicable Pennsylvania Title 55 (Chapters 6100/6400) and Title 28 regulatory requirements
Required Qualifications

High school diploma or GED equivalent

Genuine compassion for and commitment to supporting individuals with disabilities

Ability to pass Pennsylvania criminal background check, child abuse history clearance, and FBI fingerprinting as required under applicable regulations

Valid Pennsylvania driver's license and reliable personal transportation (preferred)

Current CPR/BLS certification (or willingness to obtain prior to start)

Basic written and verbal communication skills for documentation and team coordination

Ability to work independently with minimal supervision in a home or community setting

Physical ability to assist with transfers, mobility support, and other direct care tasks as needed
Preferred Qualifications

Prior experience as a DSP, home health aide (HHA), personal care aide (PCA), or caregiver in a home health or residential habilitation setting

Familiarity with Therap strongly preffered

Familiarity with Electronic Visit Verification (EVV) platforms such as HHAeXchange or Sandata

Experience working with individuals with IDD, autism spectrum disorder (ASD), or behavioral support needs

Knowledge of Pennsylvania ODP or OLTL/Community HealthChoices waiver programs

Experience implementing ISPs or behavior support strategies under the direction of a behavior specialist

Work Environment
This position operates in individual homes, residential settings, and community environments throughout Delaware County and surrounding areas. DSPs must be adaptable, reliable, and comfortable providing care in varied settings while maintaining a positive and professional attitude.

Quality Health Care Group is hiring compassionate CNAs, HHAs, and Direct Care Workers to support individuals with intellectual and developmental disabilities in home and community settings. You will assist clients with daily living activities, provide mobility support, prepare meals, perform light housekeeping, and engage clients in activities, all while monitoring their health and maintaining accurate documentation. Experience with individuals with intellectual disabilities, developmental disabilities, or TBI is a plus.
To qualify, you'll need a CNA or HHA certification, or at least 4 years of direct care experience, plus a high school diploma or GED. Experience with Therap/Axxess and ODP programs is preferred. The role requires the ability to work independently, reliable transportation, and passing a background check and health screenings.
