Joy Adult Day Health Care Center, located in Norwalk, is hiring a Korean speaking Social Worker.
Overview
We are seeking a compassionate and motivated Social Worker to join our Adult Day Health Care (ADHC) team. The Social Worker plays a key role in supporting participants and their families by providing psychosocial assessments, care coordination, and ongoing support services.
Qualifications:
- Legal to work in the U.S.
- Candidates should hold an MSW or MA(in psychology, gerontology, or counseling and have at least one year of experience), or possess a BSW with a minimum of two years of experience in a related field
- Experience in ADHC, CBAS, healthcare, or geriatric settings preferred but not mandatory
- Strong communication, organizational, and documentation skills
- Ability to work collaboratively in a multidisciplinary team
- Bilingual in English and Korean Required
- Knowledge of Microsoft Office, Word & Excel
Work schedule:
- Monday to Friday, 8:00am to 3:30pm
Responsibilities
- Conduct comprehensive psychosocial assessments upon enrollment and as required by CBAS regulations
- Participate in the development, review, and update of the Individual Plan of Care (IPC) as part of the interdisciplinary team
- Conduct assessments and quarterly reports
- Provide counseling, advocacy, and education to participants and families
- Coordinate and refer participants to appropriate community resources and benefits
- Participate in interdisciplinary team meetings, case conferences, and IPC reviews
- Ensure documentation is accurate, timely, and compliant with CBAS, Title 22, and Medi-Cal requirements

Pay: $25-30 per hour DOE
