Overview
We are seeking a dedicated and compassionate Home Health Aide & Caregiver to join our team. In this role, you will provide essential in-home support to clients with diverse needs, including elderly individuals, those with disabilities, and patients requiring specialized care such as dementia or hospice support. Your efforts will directly contribute to improving patient well-being, promoting independence, and ensuring comfort within their homes.
Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance
- Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals
- Support medication administration in accordance with medical instructions and home healthcare regulations
- Maintain accurate medical documentation using Electronic Health Record (EHR) systems and ensure compliance with HIPAA confidentiality standards
- Provide specialized care such as Alzheimer's, dementia, memory care, hospice, and end-of-life support
- Assist with transfers using patient transfer methods and equipment like Hoyer lifts or gait belts
- Perform light housekeeping tasks including cleaning, laundry, and meal preparation to promote a safe and healthy environment
Qualifications
- Prior experience working as a Home Health Aide or Caregiver in assisted living facilities, nursing homes, or home healthcare settings
- Knowledge of medical terminology, catheter care, and basic life support (BLS) including CPR
- Familiarity with EMR/EHR systems for documentation and medical records management
- Experience working with individuals with developmental disabilities or geriatrics preferred
- Ability to communicate effectively with clients and healthcare teams while maintaining patient confidentiality under HIPAA guidelines
- Physical ability to perform heavy lifting and assist with patient transfers safely
- Certification in first aid and CPR is highly desirable
- Strong organizational skills with attention to detail in medical documentation and client care plans
