Position Summary
The Home Care Coordinator is responsible for managing an assigned caseload of patients and caregivers, ensuring continuity of care, staffing coverage, attendance management, and exceptional customer service. The Coordinator serves as the primary point of contact for patients, caregivers, family members, and referral sources while maintaining compliance with company policies and contractual requirements.
Key Responsibilities
- Manage an assigned caseload of home care patients and caregivers.
- Coordinate and staff patient cases to ensure uninterrupted service.
- Conduct daily attendance follow-up and resolve missed visits.
- Monitor caregiver schedules and address call-outs, lateness, and coverage needs.
- Communicate with patients, caregivers, family members, and healthcare professionals regarding service needs.
- Document all patient and caregiver interactions accurately and timely.
- Manage holds, hospitalizations, schedule changes, and service interruptions.
- Review dashboards, reports, and outstanding tasks daily.
- Follow up on timesheets, EVV compliance, and payroll-related issues.
- Maintain compliance with company policies, contract requirements, and documentation standards.
- Participate in team meetings, trainings, and performance initiatives.

- Fluent in English and Spanish (written and verbal).
- Minimum 1 year of experience in healthcare coordination, scheduling, customer service, or a related field.
- Strong communication and interpersonal skills.
- Excellent time management and organizational abilities.
- Ability to multitask and work in a fast-paced environment.
- Strong problem-solving and critical-thinking skills.
- Proficiency with Microsoft Outlook, Teams, and basic computer applications.
- Ability to receive feedback, adapt to change, and work collaboratively within a team.
Preferred Qualifications
- Home care or healthcare coordination experience.
- Experience with HHAeXchange or similar scheduling platforms.
- Experience managing high-volume caseloads and caregiver scheduling.
