Job Summary:
The On-Call Registered Nurse (RN) provides after-hours care and support to hospice and palliative care patients and their families. This role involves responding to urgent patient needs, managing symptoms, and providing emotional support during evenings, nights, weekends, or holidays. The On-Call RN ensures continuity of care outside of regular business hours and serves as a critical resource for both patients and caregivers.
Key Responsibilities:
- After-Hours Patient Care:
- Respond promptly to patient and family calls to address urgent needs, including pain and symptom management.
- Perform assessments and interventions in patients' homes, assisted living facilities, or nursing homes.
- Administer medications and treatments as needed to provide comfort and manage symptoms.
- Provide emotional support and education to patients and families regarding end-of-life care and processes.
- Crisis Management:
- Assess and manage patient crises, including changes in condition or emergency situations, ensuring patient comfort and safety.
- Collaborate with the interdisciplinary team to address complex care needs or escalate situations as appropriate.
- Communication and Documentation:
- Maintain open communication with the interdisciplinary team to ensure continuity of care.
- Document all patient interactions, assessments, and interventions promptly and accurately in the electronic medical record (EMR).
- Notify the attending physician and team members of significant changes in patient conditions or care needs.
- Family and Caregiver Support:
- Provide guidance and reassurance to family members and caregivers during after-hours calls and visits.
- Offer grief support and resources to families as needed.
- Compliance and Standards of Care:
- Ensure care delivery complies with hospice and palliative care guidelines, as well as state and federal regulations.
- Follow organizational protocols for handling emergencies, medication management, and symptom control.
Qualifications:
- Education: Graduate of an accredited nursing program; Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Active Registered Nurse (RN) license in the state of Illinois.
- Experience: Recommended 2 years of nursing experience, with at least 1 year in hospice, palliative care, or a related field preferred.
- Certifications: Current CPR certification required; certification in Hospice and Palliative Nursing (CHPN) is a plus.
- Skills:
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and problem-solving abilities.
- Ability to work independently and make sound decisions in urgent situations.
- Proficient in electronic medical record (EMR) systems and documentation.
- Compassionate, patient-centered approach to care.
Working Conditions:
- This is an on-call position requiring availability during evenings, nights, weekends, and/or holidays.
- Frequent travel to patient homes or facilities within the service area.
- Ability to respond to urgent situations promptly and provide care in varying environments.

Pay: $70,000.00 - $90,000.00 per year
