Job Summary:
We are seeking a talented and experienced Registered Nurse to join our assessment team as a Minimum Data Set (MDS) Coordinator. As a key member of our care team, you will be responsible for conducting admission/skilled assessments to develop individualized care plans, ensuring our residents receive the highest level of personalized attention and care.
Responsibilities:
- Conduct thorough assessments of residents, including MDS assessments, to identify care needs and develop individualized care plans
- Collaborate with interdisciplinary team members to ensure effective care delivery and coordination
- Monitor and evaluate resident outcomes, adjusting care plans as necessary to ensure optimal health and well-being
- Maintain accurate and timely records of resident assessments, care plans, and outcomes
- Participate in quality improvement initiatives to enhance the overall quality of care and services
- Will be part of the on-call rotation with weekends as needed
Requirements:
- Current Registered Nurse (RN) licensure in the State of Missouri, or current multistate license
- Experience as a MDS RN, with a minimum of 2 years in a similar role
- Strong assessment and documentation skills
- Effective communication and interpersonal skills
- Ability to work collaboratively as part of a multidisciplinary team
- Strong problem-solving and critical thinking skills
