Job Summary
GC American Health LLC is seeking compassionate, dependable, and qualified Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide skilled nursing services to patients throughout Miami-Dade County.
We are looking for nursing professionals who are committed to delivering safe, high-quality, patient-centered care in the home and other approved care settings. RNs and LPNs will provide skilled nursing services in accordance with physician orders, the patient's Plan of Care, professional standards, agency policies, and applicable state and federal requirements.
Responsibilities
Depending on the patient's needs, physician orders, and the nurse's scope of practice, responsibilities may include:
- Perform nursing assessments and ongoing observation of the patient's condition.
- Monitor and document vital signs, symptoms, and changes in health status.
- Provide skilled nursing care according to physician orders and the established Plan of Care.
- Administer prescribed medications and injections within the nurse's authorized scope of practice.
- Perform medication reconciliation and provide medication education.
- Provide insulin administration and blood glucose monitoring as ordered.
- Perform wound care, dressing changes, and skin assessments.
- Provide feeding tube and enteral feeding care as ordered.
- Provide catheter and ostomy care when required.
- Monitor patients with chronic and acute medical conditions.
- Educate patients and caregivers regarding medications, disease management, nutrition, infection prevention, and safety.
- Observe and promptly report significant changes in the patient's condition to the physician, supervising RN, and/or clinical team as appropriate.
- Coordinate with physicians, caregivers, therapists, family members, and other healthcare professionals.
- Follow infection-control procedures and universal precautions.
- Maintain accurate, complete, and timely clinical documentation.
- Maintain patient privacy and confidentiality in accordance with HIPAA requirements.
- Follow all GC American Health LLC clinical policies and procedures.
RN Responsibilities May Also Include
- Comprehensive nursing assessments.
- Development and revision of nursing Plans of Care.
- Supervision and coordination of patient care as required.
Qualifications
Candidates must meet the requirements applicable to their position:
- Current and unrestricted Florida RN or LPN license.
- Graduate of an accredited nursing program.
- Current CPR/BLS certification.
- Ability to successfully complete required background screening and employment requirements.
- Home health, private duty, hospital, skilled nursing, assisted living, or related clinical experience preferred.
- Strong clinical assessment and observation skills.
- Excellent communication and documentation skills.
- Ability to work independently in a home care environment.
- Reliable transportation and ability to travel to patient locations throughout Miami-Dade County.
- Professional, dependable, compassionate, and patient-focused attitude.
- Bilingual English/Spanish is highly preferred.
Preferred Clinical Experience
Experience in one or more of the following areas is highly valued:
- Medication administration
- Wound care
- Diabetic care and insulin administration
- Feeding tubes and enteral nutrition
- Kangaroo Pump or similar feeding systems
- Catheter care
- Ostomy care
- Injections
- Chronic disease management
- Geriatric care
- Post-hospitalization care
- Private duty nursing
- Home health nursing
