Position Summary
The Hospice RN Case Manager coordinates and delivers high-quality hospice nursing care to patients in their homes or facilities in the San Francisco Bay area — overseeing the plan of care, collaborating with the interdisciplinary team, and supporting families through education and clinical guidance.

Responsibilities
- Manage hospice patient caseload in San Francisco Bay Area / Peninsula (home or facility-based)
- Develop, implement, and update individualized care plans
- Perform patient assessments, monitor symptoms, maintain clinical documentation
- Coordinate with physicians, social workers, chaplains, and other IDT members
- Provide education and emotional support to families and caregivers
- Participate in on-call rotation as needed
- Ensure compliance with hospice standards and regulations
Qualifications
- Active California Registered Nurse (RN) license (required)
- Minimum 1 year hospice or home health nursing experience preferred
- Strong clinical assessment and communication skills
- Reliable transportation and valid driver's license
- Comfortable and compassionate working in end-of-life care
- Willing to travel within the San Francisco service area
