Citizens Care & Rehabilitation Center is seeking a Memory Care Activity Professional to join our team to make a positive impact on our residents' lives through creative programming, compassionate support, and dedicated service! This individual will be responsible for overseeing the specialized activity programming for our memory care residents while collaborating with the facility's activity department.
Responsibilities/Duties
Develop and implement activity programs tailored to residents' individual interests and abilities within the specialized memory care unit.
Fosters a positive environment that encourages participation and interaction amongst residents.
Must be able to create interest in the activities and motivate residents to participate daily
Must possess the ability to exercise good judgement and make confident ethical decisions
Must be able to effectively manage time in order to complete multiple tasks
Assists in planning individual community calendars
Completes proper assessments and other resident forms; maintains other written records
Writes an individualized care plans for the residents
Follows the care plan to implement the resident’s social and recreational programs.
Assists residents and guests by transporting them to and from programs.
Communicates with nursing staff the residents and guests participating in functions for the purpose of assistance in transporting.
Attends In-Service training to maintain and improve competencies.
Must be able to communicate effectively (both orally and in writing) in English
Reports to the Nursing Home Administrator
Other duties as assigned.

Knowledge of Alzheimer’s care, memory care, dementia care and recreational therapy techniques.
Protects the privacy of residents, guests and employees.
Must have a basic understanding and empathetic approach to the needs of the resident population

Physical and Environmental Requirements
Must be able to stand, sit, stoop, push, pull, balance, reach, grasp, kneel, climb and lift up to 50 pounds.
The candidate must meet the physical requirements of the position which include transporting residents in wheelchairs to/from programs as well as on long walks. This position may also run light fitness programs while seated.

Experience
High School Graduate or GED equivalent is required.
One year experience in a recreation setting
Dementia Experience: 1 year
