Job Summary
We are seeking a dedicated and highly skilled Nurse Practitioner to join our team in a full-time capacity within a nursing home setting. The ideal candidate will possess experience in geriatric and long-term care, with a strong commitment to delivering exceptional patient-centered healthcare.
- Conduct comprehensive medical evaluations, admissions, follow-up visits, and routine assessments for long-term care and short-term rehabilitation residents.
- Develop, implement, and monitor individualized treatment plans.
- Diagnose and manage acute and chronic medical conditions.
- Collaborate with physicians, nursing staff, therapists, specialists, and interdisciplinary team members to optimize resident care.
- Respond promptly to changes in residents' conditions and provide timely medical interventions.
- Review laboratory, radiology, and diagnostic test results and adjust treatment plans accordingly.
- Maintain accurate and timely documentation in the electronic medical record (EMR).
- Participate in care planning meetings, quality improvement initiatives, and regulatory compliance activities.
- Communicate with patients and families about their care
Experience
- Proven experience as a Nurse Practitioner within long-term care or nursing home settings is required.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Athenahealth for medical documentation and patient management.
- Knowledge of infection control protocols, medication administration regulations (DEA), and clinical research practices related to elder care is preferred.

Pay: $145,000.00 - $155,000.00 per year
