Nurse Practitioner, Cardiac Focus & Primary Care seeing patients in the assisted living communities.

Responsibilities
- Conduct thorough patient assessments, including physical examinations and vital sign monitoring, to determine diagnoses and treatment plans.
- Manage acute and chronic conditions by prescribing medications, performing procedures such as suturing and IV infusions, and developing personalized care strategies.
- Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care, including discharge planning and case management.
- Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks for accurate medical documentation and coding (ICD-10, CPT).
- Perform diagnostic evaluations using laboratory tests, sonography, or medical imaging; interpret results to inform treatment decisions.
- Provide health coaching and education tailored to individual needs, including geriatrics, pediatric care, or memory care for patients with dementia or Alzheimer's disease.
- Participate in clinical research initiatives and quality improvement projects to advance evidence-based practices within the facility.
Requirements
- Valid Nurse Practitioner license with current certification to practice independently.
- Proven experience in critical care settings such as ICU or Level I Trauma Centers; hospital medicine experience preferred.
- Proficiency with EMR/EHR systems like Epic, Athenahealth, or Cerner; familiarity with ICD-9/10 coding and CPT coding is essential.
- Hands-on experience with procedures including suturing, catheterization, spinal taps, phlebotomy, and sterile processing techniques.
- Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and infection control protocols.
- Experience working with diverse patient populations such as pediatrics (including neonatal ventilator management), geriatrics (including dementia and memory care), or individuals with developmental disabilities.
- Ability to perform triage assessments in urgent care or emergency room settings; experience in telehealth is a plus.

Pay: $60,000.00 - $200,000.00 per year
