REQUIREMENTS
-
High school diploma or G.E.D. is required
-
Must be at least 18 years of age
-
One year experience in a behavioral healthcare setting preferred
-
One year experience in a clinical setting required
-
Proficient in English language
-
Certification in Basic Life Support
-
Certification in non-violent crisis intervention
RESPONSIBILITIES
-
Support and implement the Mission, Vision, and Core Values of The Maples
-
Demonstrates teamwork by carrying out routine and assigned tasks within the Maples program
-
Assists clinician with patient care tasks and therapist with therapeutic interventions
-
Assists in the provision of maintaining a safe and therapeutic milieu by monitoring and complying with program rules and policies
-
Observes patients in accordance with prescriber orders and facility policy; documents and reports findings to shift nurse immediately if behavior and/or activity is significant

Conducts and documents safety rounds per policy
-
Provides assistance in management of patients who exhibit escalating behavior, in accordance with hospital training and policy
-
Documents client activities, significant occurrences, observed behavior, vital signs, and other findings as assigned by the nursing staff and determined by facility policy
-
Assists patients in completing Activities of Daily Living (ADLs) as needed. This may include hygiene, grooming, toileting, eating, and other functions

Displays flexibility and willingness to work with others

Identifies opportunities for improvement and offers solutions; discusses with appropriate personnel (i.e., supervisors)

Consistently demonstrates vision, foresight, good judgment, and customer sensitivity

The Change Agent reports to and is accountable to the Director of Nursing.
