Position Overview:
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) Treatment Nurse Subacute to join our nursing team.
Wound Care & Treatment Administration – Provide specialized skin care treatments and administer medications per physician orders, ensuring proper wound management and infection control.
Resident Monitoring & Assessment – Conduct routine rounds to assess residents' conditions, report changes to physicians, and implement necessary interventions.
Charting & Documentation – Maintain accurate and detailed records of treatments, medications, and resident progress, ensuring compliance with regulations.
Supervision of Nursing Assistants – Oversee CNAs to ensure proper resident care, assist with scheduling, and provide guidance on nursing procedures.
Coordination of Resident Care – Collaborate with interdisciplinary teams, including physicians and therapists, to develop and update resident care plans.
Regulatory Compliance & Safety – Ensure compliance with state and federal regulations, infection control protocols, and facility policies to maintain a safe environment.
Family & Resident Communication – Educate residents and families about care plans, address concerns, and provide emotional support as needed.
Qualifications:
Valid and active California LVN License.
With Wound Care Certification is an advantage.
At least 6 months experience as LVN in subacute care and/or SNF.
At least 6 months experience in wound care, treatment, and vaccination.
Must have an active BLS through the American Heart Association.
