We are seeking an experienced, detail-oriented MDS Coordinator (RN) to oversee our Minimum Data Set (MDS) assessment process and clinical reimbursement functions!
In this key clinical leadership role, you will be responsible for completing accurate, timely MDS assessments, developing interdisciplinary resident care plans, and ensuring appropriate Medicare/Medicaid clinical reimbursement under the PDPM model. MDS & Assessment Management: Coordinate, schedule, and complete all required MDS assessments (OBRA and PPS) accurately and in strict compliance with federal and state timelines.
Care Planning: Facilitate interdisciplinary care plan meetings, working closely with nursing, therapy, social services, and dining teams to establish individualized care plans.
PDPM & Reimbursement Oversight: Monitor and optimize Patient-Driven Payment Model (PDPM) coding, ICD-10 diagnosis coding, and documentation to ensure accurate reflection of care delivered and proper clinical reimbursement.
Triple Check & Auditing: Participate in weekly Triple Check meetings prior to billing submission; perform routine audits of clinical documentation to support MDS coding.
Staff Education: Educate floor nurses and interdisciplinary team members on documentation standards, MDS coding requirements, and care plan compliance.
Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Illinois.
Experience: Minimum 2+ years of experience as an MDS Coordinator in a Skilled Nursing Facility (SNF) or long-term care setting.
Certification: RAC-CT (Resident Assessment Coordinator-Certified) preferred.
Regulatory Knowledge: Deep understanding of CMS RAI guidelines, Medicare Part A/B, Medicaid, PDPM, and IDPH long-term care regulations.
Technical Skills: Proficient with PointClickCare (PCC) or similar Electronic Health Record (EHR) systems.
