Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to develop tailored care plans.
- Administer medications via injections, IV infusions, and other routes while adhering to aseptic techniques and safety protocols.
- Provide specialized care for patients with complex needs such as wound suturing, ostomy care, catheterization, and ventilator management.
- Manage acute pain and chronic conditions through evidence-based interventions, ensuring patient comfort and safety.
- Coordinate with interdisciplinary teams to facilitate discharge planning, case management, and long-term care strategies.
- Document all patient interactions accurately within electronic health records (EHR/EHR systems), maintaining compliance with HIPAA regulations.
- Educate patients and their families on disease management, medication administration, nutrition, and self-care techniques to promote health literacy.
Requirements
- Valid RN license with current registration in the applicable state or region.
- Proven home care experience is essential; familiarity with medical office systems such as Epic, Cerner, Athenahealth or eClinicalWorks is highly preferred.
- Demonstrated expertise in diverse clinical areas including critical care (ICU/Trauma Level I & II), emergency medicine, geriatrics, hospice & palliative medicine, behavioral health, pediatrics (including neonatal resuscitation procedures), dialysis, infectious disease care, and long-term care settings such as nursing homes or assisted living facilities.
- Proficiency in advanced nursing skills such as IV insertion, phlebotomy, laboratory specimen collection/processing, tube feeding management, and aseptic technique.
- Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and documentation standards aligned with NCQA standards.
- Ability to work independently while managing multiple patient cases with excellent triage skills and critical thinking abilities.
- Experience working with individuals with disabilities or developmental delays is a plus; familiarity with trauma medicine or emergency response procedures is advantageous.
