Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to determine individualized care needs.
- Administer treatments such as IV infusions, injections, wound suturing, and medication management following strict aseptic techniques.
- Provide specialized care for patients with complex conditions including long-term care, hospice & palliative medicine, dementia and Alzheimer's care, and trauma medicine.
- Educate patients and their families on disease management, medication administration, nutrition, and safety protocols to promote optimal health outcomes.
- Monitor patient progress through regular documentation in electronic health record (EHR) systems like Epic or Cerner while ensuring compliance with HIPAA regulations.
- Collaborate with physicians, case managers, and other healthcare professionals to coordinate discharge planning and community-based services.
- Respond promptly to triage calls and urgent situations such as acute pain management or infectious disease concerns in the home setting.
Requirements
- Valid RN license with current state licensure; prior experience in home health nursing preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD-10/ICD-9 coding is a plus.
- Critical care experience including ICU or emergency room background highly desirable.
- Knowledge of advanced nursing procedures including catheterization, ostomy care, ventilator management, and sterile processing techniques.
- Strong assessment skills with the ability to perform physical examinations and interpret laboratory results.
- Ability to work independently in patients’ homes while maintaining high standards of safety and infection control practices.
