We are seeking a full-time Nurse to join our Patient-Centered Medical Home team. This role provides clinical support to providers, patients, and families to ensure efficient workflow and high-quality patient care. Evening and weekend shifts are required on a rotating basis.
Key Responsibilities:

Administer vaccines, injections (including Rocephin), and perform venipuncture

Conduct telephone triage and apply appropriate clinical protocols

Collect, process, and manage lab specimens, including in-house testing and quality controls

Maintain accurate and timely patient documentation and communication records

Communicate test results and follow-up instructions per provider orders

Initiate referrals and coordinate care, including escalation to emergency services when appropriate

Ensure compliance with safety, infection control, and patient confidentiality standards

Support providers and collaborate with the care team to ensure quality patient care
Qualifications & Skills:

Strong clinical knowledge, including emergency protocols and pediatric care

Excellent communication, customer service, and conflict resolution skills

Highly organized with strong attention to detail

Ability to multitask and adapt in a fast-paced environment

Proficient in computer systems, documentation, and phone communication

Professional, team-oriented, and able to work both independently and collaboratively

Committed to maintaining patient privacy and high standards of care
QUALIFICATIONS:

Education: Minimum high school diploma or equivalent.

Certification: Licensed Practical Nurse or Registered Nurse Certification

Experience: 1-2 years experience in the medical field preferred
