Job Title: Physical Therapist (Full-Time)

Position Summary
We are seeking a dedicated and compassionate Physical Therapist to join our team and provide high-quality rehabilitative care to patients in a home health setting. The PT will evaluate, plan, and implement individualized treatment programs to restore function, improve mobility, and relieve pain for patients recovering from illness, injury, or surgery.
Essential Responsibilities
· Conduct comprehensive patient assessments and review medical histories.
· Diagnose movement dysfunction and develop personalized treatment plans.
· Implement therapeutic exercises, manual therapy techniques, and other interventions to improve patient outcomes.
· Educate patients and caregivers on treatment plans, home exercises, and recovery expectations.
· Maintain accurate and timely documentation in compliance with regulatory standards.
· Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care.
· Monitor patient progress and adjust treatment plans as necessary.
Qualifications
· Current and valid Physical Therapist license in [State].
· Minimum of 1 year of clinical experience (Home Health experience preferred).
· Current CPR certification.
· Reliable transportation and valid auto insurance.
· Strong interpersonal and communication skills.
· Proficiency in EMR systems (experience with Kinnser preferred).
