Job Overview
We are seeking a compassionate and dedicated Certified Home Health Aide to join our team in providing exceptional in-home care to clients across diverse needs. Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor and observe patients’ conditions closely, documenting vital signs, medical changes, and behaviors accurately in electronic health record (EHR) or medical documentation systems.
- remind client to take medications as prescribed, protocols are followed in compliance with HIPAA and home healthcare regulations.
- Provide specialized care for clients with dementia or Alzheimer’s disease, including memory care techniques and behavioral management strategies.
- Support clients with end-of-life or hospice care needs by offering compassionate companionship and ensuring comfort.
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, meal preparation, cleaning, and ensuring safety protocols are followed at all times.
Skills
- Proven experience working with individuals with disabilities, developmental disabilities, geriatrics, or in long-term care settings.
- Strong knowledge of medical terminology, patient personal care assistance, and home health care best practices.
- Proficiency in EMR (Electronic Medical Records) systems and medical documentation procedures.
- Excellent patient observation skills to identify changes in condition or behavior promptly.
- Ability to communicate effectively with clients, families, and healthcare team members while maintaining patient confidentiality per HIPAA standards.
- Competence in vital signs measurement (blood pressure, pulse, respiration), CPR certification, first aid skills.
- Experience working with geriatric patients in assisted living or nursing home environments is highly desirable.
- Physical ability to perform heavy lifting when necessary and assist with patient transfers using proper techniques such as Hoyer lifts or other transfer methods.
- Knowledge of home health agency operations and familiarity with home & community care regulations.

Pay: $19.50 - $23.00 per hour
