Position Summary
The Registered Nurse Navigator coordinates and manages the care needs of patients with urogynecologic conditions, providing personalized support, education, and advocacy from diagnosis through treatment and follow-up care. This role partners closely with physicians, advanced practice providers, nurses, and ancillary departments to facilitate timely access to care, improve patient outcomes, and enhance the overall patient experience.
Key Responsibilities

Coordinate care for assigned patients and develop comprehensive, individualized care plans across the patient care continuum.

Serve as a primary point of contact for patients, families, and caregivers, providing education, guidance, and support.

Collaborate with physicians, nurses, social workers, case managers, and other healthcare professionals to ensure timely delivery of tests, treatments, referrals, and follow-up care.

Monitor patient progress and identify barriers to care, proactively addressing issues that may impact outcomes or patient satisfaction.

Advocate for patients by helping them navigate complex healthcare systems and access appropriate resources and services.

Facilitate communication among patients, providers, and multidisciplinary teams to promote coordinated, patient-centered care.

Ensure compliance with regulatory requirements, documentation standards, and organizational policies.

Support quality, safety, and performance improvement initiatives within the specialty practice.

Balance clinical needs with resource utilization and financial considerations to support efficient and effective care delivery.
Qualifications:

Requires an Associates of Nursing (ASN).

Bachelors of Nursing (BSN) preferred.

Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.

Requires that the RN has graduated from a nationally accredited nursing program.

3-5 years of experience preferred.

Requires Basic Life Support certification through the AHA.

Other advanced life support certifications may be required per unit/department specialty according to patient care policies.

Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.

Requires knowledge of personal computers using various types of word processing and database software.

Requires clinical work experience, medical terminology.

Requires understanding of medical record requirements, regulations and policies.

Medical Surgical experience is preferred but not required.
