Overview

Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
- Observe and monitor patient conditions regularly, documenting vital signs, changes in health status, and other relevant observations in electronic health record (EHR) systems while maintaining strict patient confidentiality in accordance with HIPAA regulations
- Support medication administration under supervision, ensuring proper documentation and adherence to home healthcare regulations
- Provide emotional support and companionship to patients experiencing dementia or Alzheimer's disease, fostering a calming environment through effective communication and engagement
- Assist with meal preparation and light housekeeping tasks such as cleaning and laundry to promote a safe and comfortable living environment
- Follow infection control protocols during patient care activities, including catheter care, wound management, and hygiene assistance
- Collaborate with nurses and other healthcare professionals to coordinate patient care plans and report any concerns promptly
Experience
- Prior experience working with geriatric patients in hospice, nursing homes, assisted living facilities, or home healthcare settings is highly preferred
- Demonstrated knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) or CPR certification required
- Experience working with individuals with disabilities or developmental challenges such as dementia or memory care needs is advantageous
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping is essential
- Proven ability to handle heavy lifting safely and perform patient transfers using appropriate techniques and equipment like Hoyer lifts
- Knowledge of home health care assistance regulations and best practices for providing compassionate end-of-life care in community settings
- Strong communication skills for effective client interaction while maintaining professionalism and confidentiality
