Lead Resident Care with Compassion
Woodland Estates Senior Living is seeking an experienced Licensed Practical Nurse (LPN) to serve as our Wellness Director. This leadership position is responsible for overseeing the daily clinical operations of our assisted living community, ensuring residents receive high-quality, personalized care while maintaining compliance with all state regulations.
The Wellness Director works closely with the Executive Director to lead the wellness team, support residents and families, and foster a culture of professionalism, accountability, and compassionate service.
ResponsibilitiesClinical Leadership
Supervise and support nursing, medication, and caregiving staff.
- Complete resident assessments before admission and as needed.
- Develop and update resident service plans.
- Monitor resident health and respond to changes in condition.
- Coordinate care with physicians, pharmacies, hospice, home health, and therapy providers.
- Oversee medication management and medication administration.
- Promote quality resident care and wellness.
Regulatory Compliance
Maintain compliance with Colorado Assisted Living regulations and company policies.
- Ensure accurate clinical documentation and resident records.
- Support state surveys and maintain survey readiness.
- Monitor infection prevention and control practices.
- Review incidents and implement corrective actions when appropriate.
- Maintain HIPAA compliance.
Resident & Family Support
Communicate with residents and families regarding care needs.
- Participate in care conferences and move-in assessments.
- Address concerns professionally and promptly.
QualificationsRequired
Current, active Colorado Licensed Practical Nurse (LPN) license.
- Minimum of two years of clinical experience in Assisted Living, Memory Care, Long-Term Care, Skilled Nursing, Home Health, Hospice, or a related healthcare setting.
- Minimum of two years of supervisory or leadership experience.
- Knowledge of resident assessments, medication management, care planning, and clinical documentation.
- Strong leadership, organizational, and communication skills.
- Experience using electronic health records (EHR/EMR).
Preferred
Experience in Assisted Living leadership.
- Memory Care experience.
- CPR and First Aid certification.
- Experience with Colorado state surveys and regulatory compliance.
