Join our compassionate team as a Part-time Hospice Social Worker (MSW), where you will play a vital role in providing emotional support, counseling, and resource coordination for patients facing end-of-life journeys. Duties
- Conduct comprehensive patient assessments to determine psychosocial needs and develop personalized care plans aligned with clinical ethical guidelines and social work practice standards.
- Provide individual, family, and group counseling sessions utilizing therapeutic modalities such as behavioral therapy, motivational interviewing, cognitive behavioral therapy, and clinical counseling techniques.
- Coordinate community resources to support patients’ medical, emotional, and social needs, including hospice & palliative medicine services, addiction counseling, dementia care, Alzheimer’s care, PTSD care, and specialized pediatric or geriatric support.
- Facilitate discharge planning by collaborating with healthcare providers to ensure seamless transitions from inpatient or outpatient settings to home or assisted living environments.
- Maintain accurate medical documentation and medical records using EMR (Electronic Medical Records) systems while adhering to HIPAA regulations for patient privacy and confidentiality.
- Engage in crisis intervention when necessary, providing immediate support during emergencies or behavioral health crises involving patients or families.
- Participate in interdisciplinary team meetings to promote clinical interprofessional collaboration focused on holistic patient care.
Experience
- Master’s degree in Social Work (MSW) from an accredited institution with current licensure or certification as a licensed clinical social worker (LCSW).
- Proven experience working with hospice care, end-of-life care, or palliative medicine within hospital, inpatient, nursing home, or home & community care settings.
- Demonstrated knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, Medicare regulations, and medical documentation standards.
- Hands-on experience with behavioral health practices including mental health counseling, diagnostic evaluation, crisis management, and case management.
- Familiarity with working with diverse populations across cultural backgrounds and individuals with disabilities such as developmental disabilities or autism spectrum disorder.
- Strong skills in patient assessment, discharge planning, intake procedures, and working within hospital or outpatient environments utilizing EMR systems.
- Ability to handle acute care situations confidently while maintaining professionalism aligned with social work practice standards.
- Experience working with geriatric patients and those requiring specialized dementia or Alzheimer’s care is highly desirable.
